Application for Education Benefit Forms

This urgent notice is regarding the Educational Benefits form that we need filled out for all of our full time students. This form helps with the funding to provide necessary equipment and supplies needed for assignments and to expand our course list to better suit the needs of our current and enrolling students. We really appreciate your effort in helping our program reach its full potential.

We ask you to print and fill out the Application for Educational Benefits form, along with guide that will assist you in completing it successfully.

Due to the importance of this form we ask that you fill it out and return it by December 1st, 2010.
